How to make a clump of Pediocactus simpsonii

This one got damaged at the apex, probably last year but maybe in 2016. The little ones appear to be offsets although they must be coming from very low down. I've poked about but haven't been able to find the bottom of them, so maybe they're an incredible coincidence of three seedlings appearing from nowhere.

Any idea how it got damaged? I get this sometimes with RSM, but I can't see any mite damage on your plant.

Looks more like thrips to me

I don't know what damaged it. I'd guess at mites, but nothing obvious on this particular plant. Thrips are not common on my cacti, in fact almost unheard of, but they damage a handful of Lithops every year so they're around.
