In the past several people with MAC computers have said they have had difficulty using the spreadsheet as it was created using Windows software.
Apparently if you use the Apple Numbers software you should be able to fill the form in and save it as a PDF, which can then be emailed to Peter as normal.
I don't think anyone else (Ralph maybe?) has suggested this solution. If it has been posted I can't find reference to it.
