It is not very uncommon for a Lithops to produce three leaves instead of two.
And next season the three have wilted away as happens with the leaves every year, and probably theese three will be replaced by the "normal" amount of two leaves again.

Or it might finish the job and have two heads but with a simple pair of leaves each.

Actually, trilobed Astrophytums are pretty much the same kind of swiz. They too in time tend to develop a 4th rib, and a bit later still, a fifth rib.

Incidentally, if anyone has loads of money, I have a nice 6-ribbed Crassula pyramidalis to sell you. Shame the branches dont come true...

I had a 6-ribbed C. pyramidalis a couple of years ago too, Ralph! As you say, branches just revert to normal...
