Grafted. Looks like the stock has branched and is starving it. I'm going to go with Stenocactus.

It's grafted on Harrisia jusbertii which is a pretty good stock, but it may be a graft that is failing with poor adhesion. Probably best to take it off and root it down.

Yes, the stock is Harrisia, and the offsets could have done with coming off, which they would have if the plant was mine ! the scion however is not Stenocactus (are they not Echinofossulocatus any more?)

Could the scion be Matucana madisoniorum? That was my initial gut reaction to finding out it wasn't Stenocactus.
EDIT:
Then again if the plant has somewhat distorted ribs that could explain the random arrangement of spines rather than the Matucana guess.

The scion is a genus that only occurs in Mexico, where this picture was taken.
